Challenger Space Shuttle
January 28, 1986, was a terrible day, and the end of the Challenger's life. This shuttle had many wonderful flights, but in the end, she came down in a ball of burning gasses. Challenger had a wonderful launch, but seconds would bring disaster. 73 seconds into flight, one of the O-seals in the rocket's right booster failed, burning a hole into the main liquid fuel tank. This caused the two fuel tanks to separate in mid-flight. Aerodynamics tore the ship apart from there. Fragments of equipment and passenger compartment were found on the ocean floor, after a very lengthy search. Some crew members were known to have survived the initial breakup, but none survived the compartment's collision with the ocean surface. Low temperatures, bad engineering, and miscommunication led to the loss of seven astronauts lives.
